  • Sealed transcript or diploma,
  • National Board Certification Part 1 and Part 2, which will show your exams scores,

  • Massachusetts Dental Ethics and Jurisprudence Exam score,

and a current certificate from one of the following institutions:

  • American Red Cross Cardiopulmonary Resuscitation/Automated External Defibrillation (CPR/AED) for the Professional Rescuer, or
  • American Heart Association Basic Life Support for Healthcare Providers (BLS).

Secondly, you will need to attach a passport style photo and notarized affidavit. Also, get signatures from 2 licensed dentists who are familiar with you. But they shouldn’t be your family member.

And, you should pay $660 for your Initial Dentist license. Mail all the required documents to:
